How To Use The Patient Portal

STEP 1: You will Receive a Patient Portal Email

  • After scheduling an appointment at Northside Vision, you’ll receive a “Patient Portal” email from our office. The Portal allows you to provide your medical history and complete your forms prior to your visit.

STEP 2: Log Into Your Patient Portal

  • Follow the instruction in the “Patient Portal” email to log into your Patient Portal.

STEP 3: Once You Have Logged Into Your Patient Portal

  • After accessing your Portal, click on the three horizontal lines “hamburger icon” which are located at the top left corner of the screen. This will open the menu. Once the menu has opened, click on “My Health” and a list will open along the left side of the screen. Please work your way down the list by completing the following sections:
    • Contact Info
    • Insurance & Pharmacy
    • Medications
    • Past Medical History
    • Ocular History
    • Social History
    • Family History
    • My Forms – complete any required forms

If this is your first time using our Patient Portal this process may take a few minutes, but once your information has been entered once, things will be easier in the future. Please call us at (509) 326-2772 if you have any questions.
